  1. Lie on your back.
  2. Put the whole leg on a firm slope.
  3. Heel on the pad.
  4. Nothing tucked behind the knee.
  5. Lock a height that is comfortable. Seo et al. (2020) compared 30 cm vs 10 cm after ankle ORIF: no difference in swelling; the higher position was less comfortable.

UCSF: support under the heel, not behind the knee. Brown Health (2023): rolled towel or pillow behind the calf. The LYXOMED wedge is one plane from thigh to heel, with two locked heights: 10.63 in or 7.87 in.
